- Areas of discomfort or limitation

A health disclaimer is presented before submission, and an automated confirmation is sent to the client the moment they finish. The whole flow takes about 60 seconds and is fully responsive on mobile.
From the nutrition answers, the assessment builds a display-only Nutrition Score with plain-English guidance, and flags clients who report a medical condition that affects their nutrition for referral to a qualified dietitian or physician. Nutrition output is general education for performance — not medical nutrition therapy.
